A digital twin is only useful if people trust it, and it loses that trust quickly when it shows equipment that was removed or misses a line that was added. The weak point is usually that MOCs update drawings and documents, but nobody is responsible for updating the twin.

The simplest fix is to make the twin update part of MOC closeout, so an MOC that changes equipment or connections is not complete until the twin reflects it. When the twin is built from the same drawings that the MOC updates, keeping it current becomes a matter of reprocessing the changed drawing rather than editing a 3D model by hand.
