How do you capture field failure evidence when parts are swapped without being logged?

In the field, a failed part is often swapped quickly to get equipment running again, and the old part gets thrown away before anyone records what failed or why. That leaves maintenance and engineering with no evidence to work out whether there is a pattern.

The easiest fix is to make logging the swap as quick as possible, ideally a photo of the failed part, the equipment tag and a short note captured on a phone in under a minute. If recording a failure takes longer than doing the swap, it will not happen, so the process needs to fit into the work rather than add to it.
