Most sites have years of incident and near miss reports that would be very useful before a job, but nobody has time to search them, so the lessons sit in a database rather than in front of the crew. The same kinds of incidents keep happening on the same kinds of jobs.
When incidents are linked to equipment, tasks and locations, they can be shown during the pre job review for similar work, with a short summary of what happened and what was learned. That gives the crew something specific to talk about, which makes the review more useful than a generic hazard list.
